Behavioral Health Home Services
Behavioral Health Home services through Global Health Link are designed to better coordinate care for Medicaid enrollees. BHH services include:
-Comprehensive care management
-Care coordination
-Health promotion and wellness
-Comprehensive transitional care
-Patient and family support
-Referral to community and social support services
Our professionals will identify mental health, physical health and wellness needs through a thorough assessment, which will then lead to a Health Action Plan. The plan will include different programs and appointments, which Medical Assistance covers, along with medical referrals for each program.
